An unannounced monitoring inspection was conducted from 7:45 am-8:30. At the time of entrance, 22 children were in care with 2 staff members present. Children were observed arriving to the am program and engaged in group activities. A selection of staff trainings and children records, medications and other documents were reviewed. Standard #: 22VAC40-185-60-A Description: Based on record review and staff interview, licensee has not maintained the physical and immunization record for each child. Evidence: Child #1 and #2 do not have copies of their physical and immunization record. Staff stated that she has requested copies from the public school office. Plan of Correction: I will resubmit request for health records to our registrar.
Standard #: 22VAC40-185-510-C Description: Based on a review of medications and staff interview, licensee has not followed the center's procedures for obtaining written acknowledgement from parent and physician to administer prescription medication. Evidence: Child #3 medication, Auvi-Q was on site with no written authorization from parent or physician to administer. The staff stated that the parent was obtaining the form from the physician. Plan of Correction: I will have parent obtain the authorization form from doctor.
