Technical Assistance:
The following areas of the licensing standards were discussed on October 19, 2015: 1. Diapering procedures and storage of soiled diapers - both disposable and cloth diapers. 2. Medication administration procedures - required authorization (parent and physician), storage and disposal (or return to parents). 3. Continued monitoring of injury report forms and possible separation of "incidents" as opposed to "injuries" for reporting purposes. 4. Recordkeeping - ensuring all required records are complete - including emergency contact addresses, telephone numbers, signatures, etc. The risk assessment process was discussed and copies of the risk matrix and the provider's risk profile report were provided.

Comments:
Your time and assistance were appreciated during the unannounced monitoring inspection conducted on October 19, 2015, between the hours of approximately 10:21 a.m. and 3:14 p.m. Fifty-two (52) children were enrolled and thirty-six (36) children were in the care of nine staff members during the on site inspection. One additional staff member was present and able to assist, as needed. Four children's records and eleven (11) staff records were reviewed and/or updated. Six children's medications, as well as accompanying authorization forms and logs were reviewed. Safety equipment (first aid kits, flashlights and radios), posted information (license, licensing inspection documentation, emergency evacuation diagrams and plan, emergency telephone numbers, and classroom schedules) and available documents (emergency evacuation drill record and injury report forms) were observed/reviewed. The daily schedules accurately reflected the activities provided. Lunches and snacks were provided by the children's parents. Activities observed on this date included: indoor play; arts/crafts time (making halloween decorations and masks); outdoor play; diapering, restroom and handwashing procedures; lunch; stories; naptime; infant care; and arrival/departure of children. Staff/child interactions were observed to be positive and supportive of children's growth and development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. No violations of licensing standards were cited during this inspection. A recommendation will be made for the issuance of a two-year child day center license.
