Licensing Inspector arrived at 2:00 and departed at 3:30 PM. At the time of the initial tour, there were 14 preschoolers in care with two staff. During the inspection, seven school age children arrived from school and were in the care of three staff. Additional school age children were scheduled to arrive from other schools. Children were observed participating in free play, coloring and outdoor play. Snack preparation and service were also observed.
Standard #: 22VAC40-185-60-A Description: Based on review of six children's records, it was determined that the facility did not ensure that all required elements were present in each child's record. Evidence: Child #6, date of enrollment 7/7/15, had name, address and telephone number for only one of the two required emergency contact persons. Plan of Correction: Facility responds: Second emergency contact has been added.
Standard #: 22VAC40-185-270-A Description: Based on a review of the physical plant, it was determined that the facility did not ensure that all areas and equipment of the center, inside and outside are maintained in a clean, safe and operable condition. Evidence: There was a 42 inch long piece of landscape weed-guard that was exposed in the rocks on the playground that posed a tripping hazard to the children in care. Plan of Correction: Facility responds: Cutting off the landscape weed-guard that is exposed.
Standard #: 22VAC40-185-280-B Description: Based on inspection of the physical plant, it was determined that the facility did not ensure that all hazardous substances were kept in a locked place. Evidence: There were two bottles of dry erase spray cleaner with multiple warnings on the label that were stored on the top of the back shelf of a rolling cart. Plan of Correction: Facility responds: The two bottles were put into the locked office away from the children.
Standard #: 22VAC40-185-430-C Description: Based on inspection of the playground, it was determined that the facility did not ensure that play equipment used by children had no protrusions, sharp points, shearing points or pinch points. Evidence: On the playground climber, there was a 2 and 1/8 inch protrusion where a yellow steering wheel had been attached. Staff reported that the steering wheel had come off sometime in mid-August. Plan of Correction: Facility responds: Completely take out the protrusion where the steering wheel was.
Standard #: 22VAC40-185-510-C Description: Based on a review of medication, it was determined that the facility's policies for administering medication were not effective when addressing long term medication and outdated medication. Evidence: 1. The three medications for child #6 had all expired: - Albuterol Sulfate - expired 9/15 - Epipen Jr. - expired 8/15 - Diphenhydramine - expired 8/15 2. The medication for child #1 had a parental authorization dated 8/12/15. However, there was no physician's authorization and the medication had been on site for over 10 work days. Plan of Correction: Facility responds: Medications were given back to parent of child and was asked to bring in new non-expired medication as well as asking gor physician's authorizations.
Standard #: 22VAC40-185-510-G Description: Based on a review of medication, it was determined that the facility did not ensure that they had written authorization from the parent. Evidence: There were three medications reviewed for child #6; Albuterol Sulfate, Epipen Jr, and Diphenhydramine. The authorizations for these medications were not signed by the parent. Plan of Correction: Facility responds: Parent signature on three medication forms.
