Comments:
An unannounced monitoring inspection was conducted on October 16, 2015 from approximately 12:00 PM to 2:00 PM. 4 staff were present with twenty- one children. Children were observed during nap time and staff preparation to pick up children at school. Three children records, one staff records and two injury reports were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, diapering procedures, hand washing procedures, nutrition, special feeding, injury report documentation, children's records, emergency evacuation and shelter-in-place drill documentation, health inspection report, emergency supplies and required positing. No medication is given at the center. The provider's response for the "plan of correction" was not received as of 10/30/2015 and will not appear on this Violation Notice.

Standard #: 22VAC40-185-130-A
Description: Based on 1 of 3 children's records reviewed, the licensee did not ensure the records contained an immunization record before each child can attend the center. Evidence: During the inspection conducted on October 16, 2015, the records for child #1 ( start date 4/16/13) did not have an immunization record.

Description: Based on 1 of 3 children's records reviewed, the licensee did not ensure the records contained a physical examination. Evidence: During the inspection conducted on October 16, 2015, the records for child #1 did not have a physical examination.

Description: Based on observation, the licensee did not ensure that all hazardous substances were kept in a locked place. Evidence: During the inspection conducted on October 16, 2015, in the infant classroom, there was a can of disinfectant spray on white shelf next to the changing table.

Description: Based on record review and staff interview, the licensee did not ensure there was member trained in first aid and CPR on premises during the center's hours of operation. Evidence: During the inspection conducted on October 16, 2015, the Licensing Inspector requested to review documentation of a staff members present with first aid and CPR training. Staff #2 verified that staff present do not have a current CPR and First aid.
