Comments:
An unannounced monitoring inspection was conducted on 10/30/15 between 4:15 and 6:15. The inspection was conducted with the director. The inspector spoke with staff and children. The following staff/child ratios were observed: Infant ? one staff with three children; Toddlers - one staff with five children; 2 and 3?s - one staff with eight children; 4 - 5's - one staff with ten children; 5 - 6's ? one with eight children; School age - three staff with 26 children. The children were engaged in various activities including playing with puzzles, coloring, playing with blocks and eating snack. Snack today was caramel apples and milk for the school age children and gingerbread cookies, apples and milk for the preschool children. Health inspection ? 9/1/15; Fire inspection ? 1/22/15; Last fire drill 10/23/15 and a shelter-in-place drill was conducted on 8/24/15 All required documents were posted including license, licensing notices, menu, and evacuation information. The center is enrolled in the Star program and staff received at least 40 hours of training.

Violations:
Standard #: 22VAC40-185-130-B
Description: The center failed to obtain an immunization update for one child. Updates are required once every six months for children under the age of two years.
Evidence:
A review of three children's records found that Child #2, DOB 4/22/14, had not had an immunization update since 12/14, when he began care. An update was required by 6/15.

Plan of Correction: Will get updated immun. by 11/13/15.

Standard #: 22VAC40-185-140-A
Description: The center failed to obtain a physical exam for one child within 30 days of the child beginning care. Evidence: A review of three children's records found that Child #2 did not have a completed physical exam. Child #2 began care on 1/20/15.

Plan of Correction: Will get updated physical by 11/13/15.

Standard #: 22VAC40-185-510-A
Description: The center failed to obtain a parental signature to administer an emergency medicine for one child. The doctor had signed the medication form.
Evidence:
A review of the medicine on hand at the center found that an Epi-pen for Child #4 did not have a signed authorization from the child's parent. The form was not signed by the parent.

Plan of Correction: Will have mother sign med. auth. form on 11/2/15
