Comments:
Today's visit was a renewal inspection. The inspection began at 8:45am and concluded at 3:40pm. There were thirty-nine children present with seven staff members and the Program Director was also present. During the inspection all six classrooms were observed, outdoor time was observed, both playgrounds were viewed, the red van was viewed, lunch time was viewed three medications were viewed, nap time was observed, six out of eleven staff records were viewed, three out of sixty-five children's records were viewed, arrival of school age children was observed, emergency supplies were viewed, and some policies and procedures were viewed. Your renewal application has been received in the licensing office and once the application has been deemed complete and today's inspection is completed, a recommendation regarding the license can be made. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on the inspector's review of a sample of six out of eleven staff records, the center failed to ensure a TB screening was not more a year old for a staff member upon being hired. Evidence: The record for staff member #4 has a hire date of 8/10/15 and a TB screening dated 2/26/2015. The TB screening was more than a year old upon being hired and therefore was not acceptable.

Plan of Correction: The PD will not accept TB screening for future staff if they are over a year old.

Standard #: 22VAC40-185-70-A
Description: Based on the inspector's review of a sample of six out of eleven staff records, the center failed to ensure a record had all of the required information. Evidence: The record for staff member #4 with a start date of 8/10/15 does not have two out of two required reference checks.

Plan of Correction: The PD will conduct reference checks and document the references checks in the staff member's record if the original reference checks cannot be located.

Standard #: 22VAC40-191-60-C-1
Description: Based on the inspector's review of a sample of six out of eleven staff records, the center failed to ensure a a staff member had results of a criminal history report within thirty days of employment. Evidence: The record for staff member #1 has a start date of 6/15/15 and the request was stamped received by the agency responsible for processing the request on 8/11/15 and completed on 8/25/15. The findings were not received within thirty days of employment.

Plan of Correction: The PD will ensure all background check requests are made within a staff member's first week of employment and will conduct follow-up with the agency responsible for making the request at the end of thirty days if the findings have not been received.

Standard #: 22VAC40-191-60-C-2
Description: Based on the inspector's review of a sample of six out of eleven staff records, the center failed to ensure a a staff member had results of the search of the central registry within thirty days of employment. Evidence: The record for staff member #1 has a start date of 6/15/15 and the request was stamped received by the agency responsible for processing the request on 8/3/15 and completed on 8/20/15. The findings were not received within thirty days of employment.

Plan of Correction: The PD will ensure all background check requests are made within a staff member's first week of employment and will conduct follow-up with the agency responsible for making the request at the end of thirty days if the findings have not been received.
