The following transportation standards were reviewed with the center director: Standard 550.H : The center shall prepare a document containing local emergency contact information, potential shelters, hospitals, evacuation routes, etc. that pertain to each site frequently visited or of routes frequently driven by center staff for center business (such as field trips, pick-up/drop off of children to or from schools, etc.). This document must be kept in vehicles that centers use to transport children to and from the center. Standard 580.C.5: The following information is in transportation vehicles: *Emergency numbers 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center. Additionally, diaper ointment can not be applied without written authorization from the child's parent. Refer to Standard 520.C.1
An unannounced renewal inspection was conducted on 11/04/2015 from 2:40pm - 5:10pm. The following staff to child ratios were observed: *Infants - 7 x 2 staff *Toddlers - 4 x 1 staff *2's - 4 x 1 staff *3's & 4's - 8 x 2 staff *SA - 16 x 1 staff at 4:40pm Children were observed during nap, afternoon snack, afternoon play and departure. Transportation and arrival from school procedures reviewed. Infant feeding and play reviewed. Diapering was observed in the toddler class. Additionally reviewed: emergency supplies, five children's records, five staff records, hand washing, sanitizing procedures, documentation of injuries, documentation of emergencies practice drills, the posted menu, written policies and procedures. No medication is being administered at this time.
Standard #: 22VAC40-185-560-G Description: Based on observation, the center failed to ensure that when food is brought from home the food container shall be clearly dated. Evidence: A opened can of powdered infant formula was not dated. Plan of Correction: The can of formula was dated during the inspection. All staff will be reminded to date all cans of formula when they are opened.
Standard #: 22VAC40-185-570-A Description: Based on observation, the center failed to ensure that when a child is placed in a high chair, the protective belt shall be fastened securely. Evidence: When the infant room was viewed at 4:00pm two infants were observed sitting in high chairs eating snack. Neither child had the safety belt engaged. Plan of Correction: The safety belt was engaged for both children during the inspection. Further, all staff will be retrained and reminded that anytime a child is placed in a high chair the safety belt must be securely fastened.
