The inspector arrived at 9:00 AM and departed at 12:10 PM. Breakfast service and activities were observed. Outdoor play was not observed as the children did not go outside due to fall picture day. Medication administration was reviewed. A sampling of eight children's records were reviewed. A sampling of four staff records were reviewed at the main office on 9/25/2015. The facility's plans of correction were obtained from the program director during the exit meeting prior to the conclusion of the inspection. Please contact the inspector if there are any questions.
Standard #: 22VAC40-185-140-A Description: Based upon review of eight children's records, the facility has not ensured that each child has a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: Child 4 enrolled on September 8, 2105. The facility was unable to provide documentation of a physical examination. Plan of Correction: The facility responded with the following: Get old physical for file use and get a new one completed as soon as possible.
Standard #: 22VAC40-185-270-A Description: Based upon observation, the facility has not ensured that areas and equipment of the center are maintained in a clean and safe condition. Evidence: 1. The paint above the sink in the boys bathroom and above the classroom sink in room 2 has bubbled and is peeling. 2. The paint on the window sill and along the right side of the window sash on the far left window in classroom 8 is peeling. 3. Masking and duct tape has been used to seal air leaks along the bottoms and sides of five windows in classroom 2. The glue on the tape has dried and cracked and the tape is peeling away from windows. Plan of Correction: The facility responded with the following: 1. & 2. Scrape or sand and repaint the areas. 3. remove and replace tape where needed
