Comments:
An unannounced monitoring inspection was completed on 11/5/2015. There were 18 children were in care at the center, ages two (2) years, 6 months through six: (6) years old with three staff supervising the children. The inspector arrived at the program at 9:45 and departed the program at 1:30 p.m. Five staff's records were reviewed and four children's records were reviewed. Informal interviews were held with all staff members and there were interactions with most of the children. A walk-through of the center was done. The children were observed in an inside activity morning gathering time, snack, free play, lunch, nap and departure. Discussion with staff about: new building, changes that might be occurring with building over the summer and outside time.

Standard #: 22VAC40-185-160-C
Description: Based on review of five staff's records, the center failed to ensure that all TB screening were repeated every two years. Evidence: 1. The inspector found that staff # 1 most recent TB screening was completed on 9/13/2013. .

Plan of Correction: The office staff will send the staff person to obtain a TB screening and will monitor all deadlines.

Standard #: 22VAC40-185-70-A
Description: Based on review of five staff's records, the center failed to ensure that all records were complete. Evidence: 1. The inspector found that staff # 2 did not have a job title or start date.

Plan of Correction: The office staff will add the missing information to the record. All records will be monitored for completeness.

Standard #: 22VAC40-185-440-A
Description: Based on observation of nap time, the center failed to ensure that all children w ere provided with a rest mat or a cot. Evidence: 1. The inspector found that two children did not have a rest mat. One child had a yoga mat and the other child had a sleeping bag style coverings that was used for the rest mat or the cot.

Plan of Correction: The classroom staff will inform parents that a rest mat is required for all napping children in care. If the parent does not provide a rest mat the center will provide them.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on review of five staff's records, the center failed to ensure that all backgrounds were repeated. Evidence: 1. The inspector found that staff # 4 have an updated sworn statement. Most recent sworn statement was signed on 12/12/2011. 2. Staff # 5 did not have repeated background checks completed every three years. The most recent CPS was completed on 8/13/2012, CRC completed on 7/31/2012, and sworn statement completed on 6/5/2012.

Plan of Correction: The office staff will complete the background checks ASAP and will monitor all dates.
