Thank you for your time and assistance, regarding the announced other inspection, that was conducted today, November 2, 2015. The on-site visit was conducted from 10:30 a.m. to 12:00 p.m. The inspection involved a situation that was self-reported to the Valley Licensing Office on October 27, 2015. Two staff members were interviewed during the visit. One violation was issued that pertained to the reported incident. The risk rating for the violation was noted on the supplemental page. Description: Staff A was observed using inappropriate behavioral guidance with one child. Evidence: Based on conversation with the staff it was confirmed Staff A used inappropriate behavioral guidance with one child.

Plan of Correction: Staff A was interviewed and immediately dismissed from employment. Parents in the classroom were called and the situation was discussed with each family. Staff was retrained on appropriate behavioral guidance.
