22VAC40-185 SPECIAL CARE PROVISIONS AND EMERGENCIES.
A complaint investigation was conducted with the local agency and law enforcement regarding supervision and special care provisions and emergencies on 10/7/2015 and 10/27/2015. Staff interviews were conducted. Documentation, staff and children's records were reviewed, and classroom observation was conducted. The complaint regarding sight and supervision and accident reports was determined to be valid. Please have the plan of correction and the date to be corrected by returned by 11/6/2015 at noon. Standard #: 22VAC40-185-340-F Complaint related: Yes Description: Based on review of documentation and staff interviews, it was determined that not all children under the age of ten years old were within sight and sound supervision at all times. Evidence: Child A, age ten months, was injured and had a bloody lip on 9/2/2015. The parent of Child A noticed the injury at pick up. No staff could explain how the child sustained the injury or when the injury occurred. On 10/5/2015, Child A was again injured and had a bloody lip. Staff B noticed the injury at approximately 4:30 p.m. and notified the parent at pick up. No staff could explain how the child sustained the injury or when the injury occurred. Plan of Correction: These incidents occurred with a newly mobile infant. The 9/2/15 incident was not notified or reported to the school until after the mother had been out of the car and came back. The 10/5/15 incident was reported to the mother verbally, as she was picking up minutes after the lip was seen. An accident was completed that evening and given to the parent the next morning. Staff will retrained on being more thorough with the health assessment of infant, as well as supervision.
Standard #: 22VAC40-185-550-M Complaint related: Yes Description: Based on review of documentation and staff interviews, it was determined that the center failed to maintain a written record of children's minor injuries in which entries are made the day of the occurrence. Evidence: On 10/6/2015, Child A was injured sustaining a bloody lip. Staff notified the parent at pick up, but no written record was provided. Documentation of the incident was recorded the next business day on 10/7/2015. Plan of Correction: We will retrain staff on more timely documentation. We will ask the parents to wait for the documentation to be finished if the accident has occurred directly before their arrival.
