Discussed ways to document that a staff person has viewed each child's original proof of identification. Please include the staff signature and date on all orientation documentation.

An unannounced monitoring inspection was conducted today. There were 64 children in care, with a total of 11 staff members assigned to their direct supervision. The children were engaged in a large variety of activities, including: playing turkey bingo, making turkey books, making night animal collages, enjoying free choice activities, creating Thanksgiving art, and playing outside. Standard #: 22VAC40-185-70-A
Description: Based on a review of new staff records, it was determined that three records were not complete. Evidence: Staff record #1 - #3 did not include documentation of two completed reference checks.

Plan of Correction: The reference checks were completed verbally. The results of each reference check will be documented and then placed in each employee's file.

Standard #: 22VAC40-185-240-A
Description: Based on a review of new staff training, it was determined that one staff person did not complete all required components of "First Day Orientation" training. Evidence: Staff #3 did not complete "Recognizing Child Abuse and Neglect" training.

Plan of Correction: I will make sure she completes the training.

Standard #: 22VAC40-185-270-A
Description: Based on observations, it was determined that one piece of equipment has not been maintained as necessary. Evidence: In the 2's bathroom, the diaper changing pad has torn.

Plan of Correction: I will get a new diaper changing pad.

Standard #: 22VAC40-185-280-G
Description: Based on observations, it was determined that spray bottles of cleaning solution were not labeled. Evidence: The spray bottles of cleaning solution were not labeled to indicate the contents.

Plan of Correction: The bottles will be labeled today.

Standard #: 22VAC40-185-510-C
Description: Based on a review of children's medications, it was determined that one authorization is not complete and one medication has expired. Evidence: 1. The authorization for one child's long-term emergency medication does not include a documented start and stop date for which the authorization is effective. 2. One child's emergency prescription medication expired on 10/31/15.

Plan of Correction: 1. The parent will be asked for a corrected authorization form. 2. The parent will be asked to bring a new medication.
