22VAC40-191 Background Checks (22VAC40-191)
An unannounced renewal inspection was conducted on 11/02/2015 from 9:25am - 12:05pm. The following staff to child ratios were observed: *Infants - 6 x 2 staff *Toddlers - 8 x 2 staff *2's - 15 x 2 staff *3's - 10 x 1 staff *4's - 10 x 1 staff Children were observed during morning program time, lunch and nap. Hand washing and diaper changing was reviewed. All applicable infant standards were reviewed. Additionally reviewed: injury documentation, emergency supplies, documentation of emergency practice drills, records for four children and two staff, the posted menu, and written emergency plans. *No medication is being administered at this time. Please complete the "plan of correction" for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-185-130-B Description: Based on record review, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The most recent immunizations on file for child #1 were administered on 2/16/2015. 2. Child #1 is under the age of two years and was in care on the date of inspection. Plan of Correction: Parent brought updated immunization documentation the following day.
Standard #: 22VAC40-185-260-A Description: Based on record review, the center failed to obtain an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: The most recent fire inspection on file was conducted on 9/18/2015. Plan of Correction: Fire inspection report was completed on 11/2/2015.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center failed to ensure that areas and equipment of the center shall be maintained in a safe condition. Evidence: A nail was observed protruding from the plastic molding on the side of the diaper changing table in the toddler class. The molding had separated from the back splash edge of the diaper changing table causing a nail to protrude outward 1/2 inch. Plan of Correction: Nail was removed from diaper table.
Standard #: 22VAC40-185-500-A Description: Based on observation, the center failed to ensure that staff shall wash their hands with soap and running water before and after diaper changes. Evidence: 1. A staff person in the two year old class failed to wash her hands after changing a child's diaper at 11:40am. a. Staff #1 was observed completing a diaper change at 11:40am. She removed the child from the diapering table and took off her gloves. She changed the paper on the diapering table. b. Staff #1 then placed a different child on the diapering table and proceeded to put on a new pair of gloves. c. At this time, the inspector intervened and asked staff #1 to wash her hands before changing the diaper of the second child. Plan of Correction: Staff member was retrained on diaper changing procedures.
Standard #: 22VAC40-185-550-M Description: Based on record review, the center failed to ensure that the time of parent notification was noted on children's injury reports. Evidence: 4 of 13 injury reports viewed for the month of October 2015 did not have documentation of the time the parent was notified of the child's injury. Plan of Correction: Staff was retrained on how to properly fill out incident reports.
