Technical Assistance:
Consultation was given regarding the use of and availability of the designated program space and restrooms during the programs operating hours.

Comments:
An unannounced monitoring inspection was conducted on November 13, 2015. At the time of entrance there were 14 children and 2 staff members present. The sample size consisted of 3 children's records and the emergency information and health related information for 2 staff records. Children were observed coloring, playing with blocks, and board games. The inspection began at approximately 6:50 AM and ended at approximately 9:00 AM. The center is now only using one outside modular units as a part of their program. Areas of non-compliance are noted on the violation notice. The results of the inspection and violations cited were reviewed and verified with the centers Recreation Supervisor. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return a signed copy to the licensing office within 10 calendar days of receipt. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review, the licensee did not ensure each child had a complete record with all required information. Evidence: During the inspection on November 13, 2015, the record for child #2 and child #3 did not include the second emergency contact.

Plan of Correction: The site superviosr will contact the parent for the information to have a complete record on file.

Standard #: 22VAC40-185-540-A
Description: Based on observation and staff interviews, the licensee did not ensure that in each building used by children there was a first aid kit. Evidence: During the inspection on November 13, 2015, there was not a first aid kit in the second building (modular unit).

Plan of Correction: The site supervisor will ensure the second building (trailer) has a first aid kit.

Standard #: 22VAC40-185-540-E
Description: Based on observation and staff interviews, the licensee did not ensure that in each building used by children there was one working, battery-operated flashlight and one working, battery-operated radio. Evidence: During the inspection on November 13, 2015, there was not a battery operated radio or flashlight in the second building (modualr unit).

Plan of Correction: The site supervisor will ensure the second building (trailer) has a battery operated flashlight and a battery operated radio.

Standard #: 22VAC40-185-550-C
Description: Based on observation and staff interviews, the licensee did not ensure that in each building used by children there was an emergency evacuation and shelter-in-place procedures/maps posted in a location conspicuous to staff and children on each floor of each building. Evidence: During the inspection on November 13, 2015, there was not an emergency evacuation map or emergency procedure posted in the second building (modular unit).

Plan of Correction: The site supervisor will ensure the second building (trailer) has the emergency maps and procedures posted.
