Unannounced inspection was conducted beginning at 11;30 and ending at 12:45. The inspection was conducted to investigate a complaint that was received today regarding inappropriate disciplinary actions. The Licensing Inspector interviewed three staff and the Director and the complaint was deemed to be Valid. The Director and the staff have made adjustments to the behavior management plan for this child and may include additional staff assistance. Description: Based on staff interviews, it was determined that Staff A used a forbidden action in disciplining Child #1. Evidence: Child #1 was acting out and Staff A put him in a small wooden chair and buckled him into the chair. The child could not extricate himself from the chair.

Plan of Correction: The Director stated that the staff person will be monitored and the staff person has agreed to no longer use that method of discipline.
