Comments:
An unannounced monitoring inspection was conducted from 10:25 A.M. until 1:00 P.M. At the time of the entrance, 15 children were in care with two staff members present. The sample size consisted of three children's records, two staff records and two children's medication and supporting documents. Children and staff were observed engaged in free indoor play, group educational activities, story time, hand washing, bathroom break, and during lunch and nap. Staff were observed during transition and behavioral management. During circle time children learned about butterflies and vegetables. Violations from the previous inspection were reviewed for current compliance. No repeat violations were found. Areas of non-compliance are identified on the violation notice. Description: Based on record review and interview, the center staff failed to complete the required documentation on one out of three injury reports reviewed. Evidence: 1. An injury report for child 3 dated 10/8/15 was reviewed. The report did not contain the date and time the parent was notified. 2. The lead teacher stated the injury report went home with the child the day of the injury.

Plan of Correction: In the future the center staff will ensure that all required documentation is completed on the injury report.
