Today we discussed posting of the license and public access to rest rooms used by children in care.

An unannounced inspection of the facility inside and outside was conducted from 3:06pm - 5:00pm. There were 27 children in care ages 4 - 10 years with three staff including one program administrator. The outside play space contains three pieces of climbing equipment. However, it was reported today that children are not allowed to climb on the equipment. The program director reports that there is not any medication being administered at this time. Children were observed in teacher and self-directed activities including snack, rest room routines, learning centers and homework. Four children's records and three staff emergency records were reviewed. Staff records are stored at a central location and will be reviewed at a later date. Violations in the following areas of standards were observed and may be reviewed on the violation notice; administration, physical plant and special care provisions and emergencies.

Standard #: 22VAC40-185-140-A
Description: Based on record review and interview the center failed to ensure that each child has a physical examination on file. Evidence: 1 - The record for child 1 does not include documentation of a physical examination. Child 1 has a documented first date of attendance as 12/29/14. 2 - Staff 1 confirmed that there is not a physical on file for child 1.

Plan of Correction: Center management will obtain the information from the parent.

Standard #: 22VAC40-185-40-D
Description: Based on observation and interview the center failed to ensure that the license is posted in a place conspicuous to the public. Evidence: 1 - The current license is posted on the back side of the parent information board. The backside of the board in faces the stage and cannot be seen by any clients or potential clients as the enter and leave the program. 2 - The program director confirmed that the location of the license was not visible to persons coming to and going from the program.

Plan of Correction: Center management will place the license in a location that is visible to everyone who enters the program.

Standard #: 22VAC40-185-260-A
Description: Based on record review and interview the center failed to provide the licensing representative with an annual fire inspection report. Evidence: 1 - The fire inspection report provided to the inspector is dated 5/29/14. 2 - The program director confirmed that the fire inspection provided is the most recent inspection available.

Plan of Correction: Center management will ensure that a fire inspection is obtained as soon as possible.

Standard #: 22VAC40-185-540-E
Description: Based on record review and interview the center failed to ensure that a battery operated radio is available. Evidence: The program director stated that they had not yet received batteries for the school year and therefore, the radio works only when plugged into an electrical outlet.

Plan of Correction: Center management will obtain batteries for the battery operated radio.
