Discussed paperwork and how long the center should hold on to attendance sheets and sign in/ sign out sheets.
37 children present with 11 staff in two groupings. The children are bundled up and playing outside in the rain. Playing with cars, jumping in puddles and playing on the hill, lots of activities outside. The children were observed playing freeze dance , playing with toys and eating lunch. Nap time was also observed. Today's lunch was turkey-roni, salad and pears and apples. Interaction between staff and children observed during the visit. The center has lots of supplies for the children to explore. Please call me at 703-203-8659 if you have any questions regarding this visit. This visit was started at 11:00 and ended at 1:30.
Standard #: 22VAC40-185-70-A Description: The center failed to get subsequent back ground checks for one staff. Evidence: Based on the review of 6 staff records , one staff , staff B is missing the subsequent background checks. Criminal records check was done 10/25/2012, sworn disclosure 10/15/2012 and a Child protective service checks done 2/10/2000, Plan of Correction: We will submit new background paperwork and have her fill out a new Sworn disclosure statement.
Standard #: 22VAC40-185-510-G Description: The center failed to get an original box for one medication at the site. Evidence: Based on the review of the medications, Child F has an epipen that is not in the original box with a prescription label. Plan of Correction: We will ask the parents for the medication box.
