An unannounced monitoring inspection was conducted during the morning program. There were 114 children present. The children were grouped in 9 classrooms. Ratios were met in each classroom. Documentation of the Center's annual fire and health inspections was current. A sample of 8 records was reviewed, 3 staff and 5 children's. The LI reviewed prescribed medications for two children. The center was clean and orderly. Each classroom was supplied with age appropriate materials for the children's use. Several groups were observed in the upper level activity room during open play. Dismissal of the part-day program was also observed. Staff were observed interacting with the children. Findings of the inspection were reviewed with the PD. Standard #: 22VAC40-185-510-A Description: Based on review of documentation and children's medications, the Center failed to obtain the written authorization from the parent to administer medication. Evidence: The LI reviewed medications for two children. The written authorization for medication administration for child a. was signed by the child's physician for long-term use, the parent had not signed the authorization forms for the two prescribed medications. Plan of Correction: The responsible staff will review the medication authorization form with the parent to obtain parental signature. All authorization forms will be reviewed by the PD at the time medications are brought into the center to ensure required signatures are in place.
