22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted on September 23rd 2015 between 11:00am and 12:00pm. There were 12 children with 2 direct care staff members. Children were observed solving puzzles, washing hands, and eating lunch. The first aid kit, evacuation drills, the physical space, supervision, and samplings of staff and children's records were observed and inspected. Standard #: 22VAC40-185-160-C
Description: Based on record review, 1 of 2 staff records did resubmit Tuberculosis test results at least every two years from the date of the first initial screening. Evidence: Staff #1 last had a tuberculosis screening on August 19th 2013.

Plan of Correction: Staff #1 had their TB test updated. In the future staff will have their TB results updated every 2 years.

Standard #: 22VAC40-185-60-A
Description: Based on record review, 3 of 3 children's records were incomplete. Evidence: Child #1 and #3 did not have completed proof of identification completed. Child #1 did not have the first date and last date of attendance listed Child #2 and #3 did not have the first date of attendance listed.

Plan of Correction: * Child #1 and #3 had "proof of identification" record properly completed and verified by staff in the child's file * Child #1 had their first and last date of attendance recorded in their registration packet *The first date of attendance for Child #2 & #3 were added to their registration packets * In the future staff will ensure that first and last dates of attendance are properly marked on registration packets (not just financial agreements) and that all fields of proof of identity records are completed properly

Standard #: 22VAC40-185-540-C
Description: Based on observation the first aid kit was incomplete. Evidence: The first aid kit contained 1 triangular bandage. The standards require to have 2 or more available.

Plan of Correction: More triangular bandages were provided to the South Riding facility. The first aid kit now has more than 2 triangular bandages available. We will ensure in the future that the kit always has at least 2 triangular bandages.
