Technical Assistance:
1. Discussed uneven floor in one classroom and lobby, and subsequent crack of floor in classroom. Director reported it has been present for years and not a new development. Recommend ongoing monitoring for further deterioration. 2. Reviewed documentation of written injury reports. Time parent notified should be a specific time of day, not "at pick-up." 3. The previously submitted Allowable Variance was approved by VDSS.

Comments:
Conducted an unannounced monitoring inspection. Observed 7 children + 2 direct-care staff + a contract employee. Staffing has remained consistent for a period of years. Children were observed departing for the day, taking naps, and participating in a dance class. The building was found to be clean, safeguarded appropriately, and sufficiently supplied with toys and equipment for the children. The outdoor playground was inspected. Required documents were posted. Record keeping violations found. Inspection completed 1:55-3:10pm.

Standard #: 22VAC40-185-160-A
Description: Based on interviews conducted, it was determined that an employee of an independent contractor did not have documentation of a negative TB screening. Evidence: The Director reported that the school has no employee record for a contract staff person observed to be working directly with children today, to include a TB screening.

Plan of Correction: Joy of Dance has all their employees TB screened I have requested a copy of this screening for the instructor who comes to our school. [sic] Forms received on 12/1/15

Standard #: 22VAC40-185-60-A
Description: Based on 2 children's records reviewed, it was determined that 1 record did not include complete emergency contact information. Evidence: Child #14 - 0 of 2 emergency contacts did not include addresses.

Plan of Correction: Requested parent to update form with addresses

Standard #: 22VAC40-185-230-B
Description: Based on observations made and interviews conducted, it was determined that an employee of an independent contractor was not maintained in sight and sound supervision of a school staff member. Evidence: A contract employee was observed working alone with 6 children in a classroom with the door shut, while the nearest staff person was in the ratios of another classroom and not able to provide sight and sound supervision of the contract employee. The Director reported that the school has no employee record for the contract staff person, to include qualifications needed to for the individual to be included in the ratios and to be working alone with the children.

Plan of Correction: Requested all documentation from Joy of Dance to make sure their teacher can be in a classroom with our students Forms received on 12/1/15

Standard #: 22VAC40-185-270-A
Description: Based on observations made, it was determined that a piece of equipment was not maintained in a safe condition. Evidence: A plastic climbing cube on the playground had a crack approximately 12" in length on the top of the platform. The crack posed as a pinch point hazard.

Plan of Correction: Playground climbing cube will be removed from playground. ASAP by Dec 10 2015

Standard #: 22VAC40-191-40-D-1-C
Description: Based on 4 staff records reviewed, it was determined that 1 record did not include current background checks. Evidence: Staff #4 - The previous Sworn Disclosure Statement expired in August 2015, and the previous Criminal Record Check expired in September 2015.

Plan of Correction: New Sworn Disclosure signed. Awaiting Criminal record checks return.

Standard #: 22VAC40-191-40-D-3-A
Description: Based on interviews conducted, it was determined that background checks have not been obtained for an employee of an independent contractor. Evidence: The contract employee was observed to be working alone with children today, and the Director reported that background checks have not been obtained, viewed, or verified. This includes a Sworn Disclosure Statement, Criminal Record Check, and Central Registry Check.

Plan of Correction: Requesting all documentation from Joy of Dance who conduct background checks on their staff. Forms received on 12/1/15
