Comments:
An unannounced monitoring inspection was conducted today. All groups are in ratio during the inspection. One staff and four children's files were reviewed. See violation notice for missing information. The facility has a new extended day director. Children were observed eating snack and during transition. Medications were reviewed. Violations:
Standard #: 22VAC40-185-60-A
Description: Based upon a review of files, children's files are incomplete. Evidence: 3 of 4 children's files did not contain documentation of previous care. (Children 1,2,3).

Plan of Correction: Parents will be asked to complete the missing information.

Standard #: 22VAC40-185-510-G
Description: Based upon a review of medications, two authorizations are not on acceptable forms. Evidence: Two forms available for review are on Fairfax County Public School forms. (Child 1,4)

Plan of Correction: The parent has been notified. New forms will be filled out for the medications.
