An unannounced monitoring inspection was conducted today, November 20, 2015 to determine the center's compliance with licensing standards. The inspector arrived at 1:50 pm and concluded the inspection at 4:00 pm. Upon the inspector's arrival, children were napping. Later children were observed engaged in various activities. The preschool children had circle time where they listened to a story and sang songs before going outside to play. The two year old classroom had center time with small groups of children playing with blocks, cars, house keeping and lacing cards. Infants were observed having snack and playing on the floor with toys. Staff interacted with children in a positive manner. Children in the toddler room played with push toys and small riding toys. Children danced with staff to music that was playing. Diapering was observed in this classroom. Later school age children arrived via public school bus and center van. Children had snack before starting homework and center time. The playground area was inspected. The classrooms are decorated with the children's artwork and are equipped with age and stage appropriate materials. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. A poster was posted with pictures of "the elf on a shelf" on vacation with a note to the children saying he would arrive at the center soon in preparation of the holiday. The center's first aid kit and emergency supplies were inspected and found complete. The center will be hosting a Thanksgiving feast for the children next week.
Two employee records and five children's records were reviewed during today's inspection.
Emergency medications and authorizations were reviewed. Two staff have current EMAT certification.

The last documented evacuation drill was held on 11/09/2015. A shelter-in-place drill was documented on 03/12/2015. A second shelter-in-place drill will need to be completed before the end of the year as two shelter-in-place drills are required annually.
Today the following ratios were observed:
Pre-K: ( 4 and 5 year old) 13 children with 2 staff.
Toddlers: 10 toddlers with 2 staff.
Preschool: (2 and 3 year old) 12 children with 3 staff.
Infants: 5 infants with 2 staff.
School age children: 36 children with 2 staff.
The monthly menu was posted and today's lunch consisted of: fish sticks, tatortots, mixed fruit and milk.
The violations from the previous inspection on April 14, 2015 were checked for corrections. There were no repeat violations found during today's inspection. If you have any questions regarding this inspection, please contact the licensing inspector at (804)662-9758.
Standard #: 22VAC40-185-60-A Description: Based on review of children's records, The facility did not ensure all required information was obtained for each child's record. Evidence: The record for child #4, did not have the address listed for one emergency contact. The name, address and phone number for two emergency contacts is required. Plan of Correction: Per the Director: I will have the parent complete the missing information for the record today at pick up time.
Standard #: 22VAC40-185-70-A Description: Based on review of staff records, the facility did not obtain all required information for each staff's record. Evidence: The record of staff #2, with a hire date of 11/02/2015, did not have two references as to character and reputation completed before employment nor was there written documentation to demonstrate that the individual possesses the education as required by the job position. Plan of Correction: Per the Director: I will update staff #2's record, collecting the missing information.
Standard #: 22VAC40-185-500-B Description: Based on observation, the facility did not ensure a nonabsorbent surface for diapering or changing was used Evidence: The changing table mat in the toddler room had a tear that was approximately two inches long exposing the the foam of the mat creating a non-absorbent surface. Plan of Correction: Per the director: I will replace or recover the diaper changing mat in the toddler room.
Standard #: 22VAC40-185-510-C Description: Based on review of medications, the facility did not ensure procedures included methods to prevent the use of outdated medication. Evidence: The medication for child #7 had a valid expiration date however the medication had been recalled by the manufacturer. The medication was still on site and had not been returned to the parent. The Director was aware of the recall and had addressed it with the parent. Plan of Correction: Per the Director: I will have the parent provide updated medication and return the recalled medication to them.
