An unannounced monitoring inspection was conducted today. There were 32 children in care, with a total of 4 staff members assigned to their direct supervision. The children were observed reading books, doing their homework, eating snack, and preparing to make mini chocolate pies for their afternoon cooking project. The children were served graham crackers, bananas, and milk for snack today. This inspection was completed between approximately 3:10 pm and 4:05 pm. Thank you for meeting with me today. Standard #: 22VAC40-185-60-A Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: 1. Child record #2 does not include a physical exam report. 2. Child record #3 does not include: a physical exam report; documentation to indicate that the parent's have conducted an annual review of the child's contact information. Plan of Correction: I will get copies of each physical from the school nurse. I will ask the parent to review the contact information in the file.
Standard #: 22VAC40-185-280-B Description: Based on observations, it was determined that two potentially hazardous substances were not locked. Evidence: In the SACC classroom trailer, two bottles of spray paint were stored inside an unlocked cabinet. Plan of Correction: The cabinet will be locked.
Standard #: 22VAC40-185-510-A Description: Based on a review of children's medications, it was determined that two medications were on site without authorization forms. Evidence: One child's prescription medication, and non-prescription medication, were on site without completed medication authorization forms. Plan of Correction: I will get a copy of each medication authorization form.
