Comments:
Conducted an investigation into allegations that the center did not obtain an asbestos inspection of the building. Interviews conducted and records reviewed. There was a preponderance of evidence to support the allegations therefore this complaint is deemed to be valid/founded. Complaint related: Yes
Description: Based on records reviewed, it was determined that the licensee submitted inconsistent asbestos statements. Evidence: 1. The building was built prior to 1978. 2. A written statement has not been submitted from a individual licensed in Virginia as an asbestos inspector and management planner as required by Code of VA 63.2-1811, documenting the presences and management of asbestos containing materials in the building. 3. On August 4, 2010, a document was submitted by the licensee indicating that the building was built prior to 1978, and that an asbestos inspection and management plan would be submitted. 4. On October 21, 2010, a document was submitted by the licensee indicating that the building was built after 1978, and subsequently does not require an asbestos inspection.

Plan of Correction: An inspection for asbestos was conducted on December 2, 2015. The program is waiting on report, in order to determine outcome and possible follow up needed. A date to be corrected can not accurately be provided.
