Technical Assistance:
Discussed the following with the administrators: - completed background checks - food intolerances and actions to take - reminder to have annual fire inspection completed.

Comments:
An unannounced monitoring inspection was initiated today at 2pm. The physical plant, 5 staff files, 4 children's files, caregiver training, children's activities, medications, emergency procedures, drills and supplies were inspected today. There were 20 children under the supervision of 4 staff. During the inspection, children were observed working on spelling words, reading books, learning about geography, taking naps and listening to storytime. The interactions between the children and staff were seen to be positive and constructive in nature. The center does not provide lunches, as the children bring this from home daily. Standard #: 22VAC40-185-160-A
Description: Based on file review, 2 out of 5 staff files do not have a TB test completed within 21 days after employment for all staff. Evidence 1. Staff #2 and #4 do not have a TB test/screening on file.

Plan of Correction: These staff have had this completed. They will look for the paperwork and keep them on file.

Standard #: 22VAC40-185-160-C
Description: Based on file review, 2 out of 5 staff files do not have a TB test/screening completed at least every two years. Evidence 1. Staff #1's most recent TB test/screening was completed on 10/25/13. 2. Staff #3's most recent TB test/screening was completed on 8/23/13.

Plan of Correction: These staff will complete this again.

Standard #: 22VAC40-185-60-A
Description: Based on file review, 1 out of 4 children's files does not have all required information. Evidence 1. Child #1 has "pistachios and cashew nuts" listed under allergies or intolerances to food. However, there is no documentation of what (if any) action is to be taken.

Plan of Correction: The director will ask the parents to provide this information.

Standard #: 22VAC40-185-70-A
Description: Based on file review, 2 out of 5 staff files reviewed does not have all required information in her record. Evidence 1. Staff #2 and #5 do not have the following documentation on file: - name, address and telephone number of a person to be notified in case of an emergency - written information to demonstrate that the individual possesses the education required by the job position. - information, to be kept at the center, about any health problems which may interfere with fulfilling the job responsibilities. 2. Staff #5 does not have two or more references as to character and reputation as well as competency checked before employment began.

Plan of Correction: The director will have this information to keep on file.

Standard #: 22VAC40-185-510-C
Description: Based on observations, the center did not follow all procedures for administering medications. Evidence 1. Child #5 has Benedryl as an emergency medication at the center. This expired September 2015.

Plan of Correction: The director will ask the parents to bring in a replacement.

Standard #: 22VAC40-185-550-D
Description: Based on file review, a minimum of two shelter-in-place practice drills were not completed every year. Evidence 1. There was no documentation of a shelter-in-place drill having been completed for the 2014-2015 school year.

Plan of Correction: These will be completed.
