An unannounced monitoring inspection was conducted with center director and center staff. There were 29 school age children present with 3 staff supervising. The children were involved in a variety of age appropriate activities including art projects for an upcoming winter party, board games, and Legos. Four emergency medications for four children were reviewed and are in compliance. Two staff have certification of Medication Administration Training. Standard #: 22VAC40-191-40-D-1-B Description: Based on review of three staff files, the facility failed to obtain a criminal history background check for each staff before 30 days of employment at the facility ends. Evidence: Staff B (date of hire 3/27/2015) did not have documentation of a criminal history record in the file on site. Plan of Correction: Administrative staff e-mailed documentation of the completed background check on 12/11/2015.
