The licensing inspector conducted an unannounced renewal inspection that began at the center at 9:30am and concluded with staff and board officer's records reviewed at the central office at 2:45pm. There were forty children in care: three infants with two staff, six toddlers with two staff in the toddler one room, five toddlers with two staff in the toddler two room, fourteen preschool children with two staff in the part-year preschool room, and twelve preschool children with two staff in the full-year preschool room. The children were observed at story time, listening and moving to music, at circle time, in small group activities, and during lunch. Five staff, four board officers and six children's records were reviewed. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-191-40-D-1-C Description: Based on record reviews, the center failed to obtain a repeat background check on a board officer. Evidence: 1. The inspector reviewed four board officer's records. One of four records reviewed (board officer record #3) contained a search of the central registry that was over three years old. Board officer record #3's search of the central registry was dated 08/29/12. The sworn statement and criminal history record check reports were current. Plan of Correction: The Human Resource Specialist submitted a request for a new search of the central registry for board officer #3's record, but the request was returned because it was incomplete. Additional information was added to the request and resubmitted, but to date it has not been received. The Human Resource Specialist will monitor the dates on the background checks to ensure repeat checks are obtained every three years.
