22VAC40-185 STAFFING AND SUPERVISION.
An unannounced complaint inspection was conducted on October 16, 2015 in response to information sent to the licensing office concerning supervision of children. Based on information gathered from interviews, observation and document review there is a preponderance of evidence to substantiate the allegation. The complaint is valid. Standard #: 22VAC40-185-340-A Complaint related: Yes Description: Based on information gathered during this investigation, the facility did not provide care and protection for all enrolled children. Evidence: 1. Based on interviews with staff, children and witnesses and a review of documentation, on October 13, 2015 between 6:30 am and 7:00 am, child #1 (who is three years of age) found a pair of adult scissors and cut the hair of child #2 (who is two years of age). Plan of Correction: All child scissors will be kept in secure cabinet. Adult scissors will be kept in a wide mouth jar with screw lid and will be kept out of reach of children.
Standard #: 22VAC40-185-340-G Complaint related: Yes Description: Based on information gathered during this complaint investigation, the facility did not have sight and sound supervision of each child in care. Evidence: 1. In the early morning, on October 13, 2015 staff #1 and 2 were on duty. 2. Staff #2 went downstairs to retrieve supplies. 3. Staff #1 was standing in the classroom doorway to unlock the front door as parents and children arrived. This required stepping away from the classroom door to press a button. 4. Staff #1 did not see child #1 with the scissors, nor did she see the child cut child #2's hair. Plan of Correction: Staff will not leave designated area or stand in doorway. Staff will be in sight and sound of children. Staff will wait for a replacement is completely in room and ratios will be maintained. All child scissors will be kept in secure cabinet. Adult scissors will be kept in a wide mouth jar with screw lid and will be kept out of reach of children.
