An unannounced monitoring inspection was conducted on December 11, 2015. The center has a total enrollment of 13 children. Upon arrival to the facility there were 13 children present with two staff members. The children were observed creating Christmas trees, playing in activity stations, staff redirecting children and bathroom procedures. The children were also observed outside cleaning up center time activities. One staff record, three children's records and one medication documentation was reviewed. Exit interview was conducted with the Program director.
Standard #: 22VAC40-185-160-C Description: Based on 1 of 1 staff records reviewed, the licensee did not ensure staff had documentation of tuberculosis screening within the past 2 years. Evidence: During the inspection conducted on December 11, 2015, staff #1's last T.B. test was completed on October 23, 2013, which is more than 2 years old. Plan of Correction: The staff members had TB test taken on December 16, 2015. Documentation was sent to licensing office.
Standard #: 22VAC40-185-60-A Description: Based on 2 of 3 children's records reviewed, the licensee did not ensure each child enrolled had a record which contained all required information. Evidence: During the inspection conducted on December 11, 2015, the following records did not contain all required documentation: -Child #1 did not have address for one of the parents. -Child #3 did not have one parent's employer's phone numbers. Plan of Correction: The children's documentation was sent to licensing office with corrections
