22VAC40-191 Background Checks (22VAC40-191)

An unannounced inspection was conducted today by the licensing inspector. The center owner/director arrived at the center shortly after the onset the inspection. Some but not all standards were checked today. There were a total of 10 children present with four staff. The toddler/two's were observed doing circle time, listening to stories being read, did a table top activity, had lunch and began nap time. The 3/4 yr old children were reviewing numbers, colors the seasons using flashcards and were coloring related ditto sheets. This group was also observed during lunch and nap time. All the children sat together during lunch. It's suggested that you may want to consider feeding the younger children first as they become tired earlier. You may also want to consider splitting up the toddler/2's group at times to do separate age and stage appropriate activities. The children would have more space for individual activities and it would allow for use of a larger variety of materials. Materials may be rotated for use by the children and accessible for use. Even young children can learn to take out and put away toys/learning materials. Reminders: Staff without medication training may administer over-the-counter topical skin gel, cream, or ointment. Other over the counter medications require a MAT trained staff person administer the medication. It's suggested that you play music that does not have activities such as clapping or touching your nose while eating. Approximate time on site: 10 am - 1:30 pm

Standard #: 22VAC40-185-140-A
Description: Based on review of a sample of childrens records, the facility did not obtain documentation of physical examination. Evidence: Child #2 who has been in care more than 30 days did not have documentation of a physical exam on file.

Plan of Correction: I will talk to parents today to provide the documentation today.

Standard #: 22VAC40-185-160-A
Description: Based on review of a sample of staff records, the facility did not ensure documentation of a tb test/assessment. Evidence: Staff #1 did not have documentation of a tb test required within 21 days of the start date.

Plan of Correction: The staff person will be required to obtain the tb documentation.

Standard #: 22VAC40-185-80-A
Description: The facility did not ensure a daily attendance record was completed. Evidence: The attendance form in use did not include the name of each, but the name of person signing the child/children in or out. Daily attendance for each child is required to be documented either by the staff or person bringing the child to the center.

Plan of Correction: We will develop a different form with the names of the children and make sure they are signed in daily.

Standard #: 22VAC40-185-240-A
Description: Based on review of a sample of staff records, the facility did not ensure staff receive initial training and orientation by the end of the first day of assuming job responsibilities.

Plan of Correction: I will obtain documentation of the information and retrain if needed as soon as possible.

Standard #: 22VAC40-185-510-A
Description: Based on observation, the facility did not ensure authorization for use of an over the counter topical ointment. Evidence: There was no written authorization for use of an over the counter topical ointment for a child in care.

Plan of Correction: I will talk to parents today to provide the documentation today.
