Comments:
Today's visit was a routine monitoring inspection. The inspection began at 9:15am and concluded at 1:00pm. There were forty-four children present with eight staff members and three additional staff present in the building who were not counting in staff to child ratio. During the inspection all five classrooms were observed during various activities, four medications were present and viewed, four out of sixteen staff records were reviewed, five out of ninety four children's records were reviewed, the outdoor playground was viewed, lunch was observed, emergency supplies were viewed, nap time was observed, and some policies and procedures were reviewed. Discussion took place regarding the preparation of hotdogs for children three years of age and under and the Administrator was referred to the CDC technical assistance manual. Standard #: 22VAC40-185-510-F
Description: Based on the inspector's review of four medications present at the center, the center failed to ensure a prescription mediation was in the original container. Evidence: The inspector observed a single Epi-Pen that was not in the original container. The pen was stored in the plastic sleeve and was not longer in its original box.

Plan of Correction: The Administrator for the program will contact the parent and request that the parent provide the original box for the Epi-Pen. All future medications will not be accepted to the center unless they are in their original container.

Standard #: 22VAC40-185-560-F-4
Description: Based on the observations at lunch time, the center failed to ensure children three years of age were not offered foods considered to be choking hazards. Evidence: The inspector observed lunch time for the two year old classroom and two classrooms of three year old children. The two classrooms with three year old children were served whole hot dogs on a bun. A whole hotdog could pose a potential choking hazard and should be prepared in a manner that prevents choking for children three years of age and younger.

Plan of Correction: The kitchen staff will be trained in preparing foods that pose a potential choking hazards for children three years of age and younger.

Description: Based on the inspector's review of a sample of four out of sixteen staff records, the center failed to ensure a portion of the sample had completed criminal history background checks within thirty days of employment. Evidence #1: The record for staff member #1 hire date of 9/21/15. The criminal record check was stamped received by the responsible agency 10/26/15 and completed on 11/2/15. The background check was not completed within thirty days of employment due to the delayed date of request. Evidence #2: The record for staff member #3 hire date of 9/30/15. The criminal record check was stamped received by the responsible agency 10/26/15 and completed on 11/2/15. The background check was not completed within thirty days of employment due to the delayed date of request.

Plan of Correction: Background checks will be requested within the staff member's first week of employment.

Standard #: 22VAC40-191-60-C-2
Description: Based on the inspector's review of a sample of four out of sixteen staff records, the center failed to ensure a portion of the sample had completed search of the central registry background checks within thirty days of employment. Evidence #1: The record for staff member #1 hire date of 9/21/15. The criminal record check was stamped received by the responsible agency 10/19/15 and completed on 11/18/15. The background check was not completed within thirty days of employment due to the delayed date of request. Evidence #2: The record for staff member #3 hire date of 9/30/15. The criminal record check was stamped received by the responsible agency 10/19/15 and completed on 11/18/15. The background check was not completed within thirty days of employment due to the delayed date of request.

Plan of Correction: Background check requests will be made within one week of a staff member's date of hire.
