Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection December 23, 2015 from approximately 10:15am to 12:45pm. The following ratios were observed with a total of 36 children present during the inspection:
Infants - 3 children with 1 staff
Toddlers - 4 children with 1 staff
Twos & Threes- 8 children with 2 staff
Fours & Fives (Pre-K) - 9 children with 2 staff
School Age ? 12 children with 1 staff

During the inspection, the Infants were observed playing on the floor with toys, eating and sleeping in their cribs. The Toddlers were observed playing with blocks and eating lunch. The Twos & Threes classroom was observed playing with play-doh, making a phone call to Santa Claus, singing songs about snowmen, eating lunch and resting at naptime. The Fours and Fives classroom was observed having a book read to them, washing their hands and eating lunch. The School Age children were observed playing board games, playing in the dramatic play center, playing on the air hockey table and building with blocks. Lunch consisted of spaghetti, mixed vegetables, peaches and milk. All areas of the center including classrooms, hallways, bathrooms, and kitchen were inspected. The playgrounds were not inspected due to weather conditions. The center was equipped with toys and supplies and items were available to the children.

The following information was reviewed: emergency supplies, injury reports, fire and shelter-in-place drills, menu, daily sheets, parent agreements, and staff training information. Management stated medication is not administered at this time.

Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, one record did not contain documentation of a physical examination. Evidence: During the inspection, the licensing inspector observed the record for Child #1 (start date 08/03/15) did not contain documentation of a physical examination by or under the direction of a physician.

Plan of Correction: Per director - I have contacted the family multiple times requesting a physical. I will contact the family again today to let them one is needed to complete the child's record.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevented access by children. Evidence: During the inspection, the licensing inspector observed two containers of disinfectant in an unlocked cabinet over the hand washing sink in the Toddler's room.

Plan of Correction: Corrected during the inspection.

Standard #: 63.2-1720-F
Description: Based on observation, interviews and review of staff records, the facility permitted one staff member who did not have a completed original criminal clearance to have direct contact with children without the required supervision. Evidence: During the inspection, the licensing inspector observed Staff #3 (hire date 12/18/2015) supervising eight children in the Two/Three Year Old classroom. The staff member was alone in the classroom with the children during naptime. The record for Staff #3 did not contain a completed criminal history record. Management staff stated Staff #3 usually has another staff member working with her. Staff are not permitted to work in a position that involves direct contact with a child receiving services until an original criminal history record has been received, unless the staff remains within sight and sound / direct supervision of another staff for whom a background check has been completed.
