Technical Assistance:
Technical assistance today included physical environment; first aid kits; parent notification; posting of previous/current Violation Notices from DSS.

Comments:
An unannounced monitoring inspection took place today from 1 - 3:20 pm. Eight classrooms were present today, and all were within ratio. I observed naptime and the beginning of snack. Violations:
Standard #: 22VAC40-185-270-A
Description: Some areas of the center were found not to be maintained in a clean, safe and operable condition today. Evidence: 1. Outdoors: There were several pieces of cracked siding observed with sharp edges accessible at child height. The wooden gates around the two air conditioning units were not sturdy and appeared to be falling inward.. The wooden retaining wall in the toddler playground had sharp splintering areas accessible at child height. There were several plastic toys outdoors today that were observed to have sharp or cracked plastic. The screen from the 1/2 day preschool room was broken and coming off the window. 2. In one classrooms at child height, cabinet doors to a diapering area were unlocked or unlatched, leaving plastic bags accessible.

Plan of Correction: 1. Areas if sharp siding will be covered with duct tape today. A work order has been put in for the wooden retaining wall. We will contact maintenance about the gate around the a.c. and the screen. 2. The bags will be stored in another area.

Standard #: 22VAC40-185-280-B
Description: During today's inspection, hazardous substances were observed not to be kept in a locked place as required in several areasvidence: 1. Plastic spray bottles labeled "Supercontact Cleaner" were observed hanging from door hinges in two of the children's bathrooms; another was observed hanging unlocked in the infant/toddler areas when not being used. 2. In the 16 month old room, two bottles of cleaners were accessible under the unlocked diaper table. 3. In another room, children's diapering ointment was accessible under the unlocked diaper table. 4. The kitchen was observed to be unlocked twice during the inspection, and chemicals were observed in storage unlocked. 5. In the first aid kit, 5 packages of Tylenol were observed to be stored in an unlocked area. 6. On the playground, the shed was observed to be unlocked, and inside were building-grade paints and other materials.

Plan of Correction: Bottles were removed and staff will be trained about locking cleaners. The plastic lock on the diapering table cabinet was not working, so a work order will be put in. A sign will be put up to remind staff to keep the kitchen locked. The tylenol in the first aid kit was dispose of. A lock will be put on the shed today.

Standard #: 22VAC40-185-440-E
Description: During today's inspection, there were cots that were observed not to be spaced as required. Evidence: In one classroom, two children's cots were less than 12 inches apart. In another class, children's cots were between the lockers and tables with no side space open at all times to allow for passage as required.

Plan of Correction: We will move talk to the teachers about this.

Standard #: 22VAC40-185-510-G
Description: A review of medications today found that one child's prescription medication did not have a prescription label attached. Evidence: The emergency allergy medication on site today for Child # 1 did not have a prescription label available.

Plan of Correction: We will give the Epipen back to the mother this evening.
