Comments:
An unannounced Monitoring inspection was conducted in the Center on December 4, 2015. The LI observed 15 children and 3 staff members present. As reported, 18 children are currently enrolled in the program. The children's ages range from three to five years. The LI observed the children during varied activities: outdoor play, hand washing procedures, the lunch meal, nap/rest time. Staff was also observed reading a book to the children. A sample of 4 children's records was reviewed. A prescribed medication and written parental authorization was also reviewed for a child. A discussion was held with the Lead Teacher regarding posting of the most recent inspection report. Please place the report lower on the information board so that parents and the public may view the report. A sample of 2 staff records were reviewed on December 3, 2015 at TAP's main administration office. Findings of the inspection were reviewed upon exit. The on site inspection concluded at approximately 2:00PM. Thank you.

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on documentation reviewed, the Center failed to obtain a current annual fire inspection. Evidence: The LI reviewed the posted fire inspection which was dated 11/06/2014. Staff reported that was the most recent fire inspection for the Center.

Plan of Correction: The Lead Teacher will contact the responsible fire official to follow-up and will request an inspection.
