An unannounced monitoring inspection was conducted from 8:00 a.m.-9:30 a.m. with the director. 30 children, ranging in age from 18 months to 5 years, were in the care of four staff. Children were observed playing with puzzles, building with Legos, reading a book, and building with gears. Morning snack today is whole grain cereal and fruit juice. Both the health and fire inspections are up-to-date. Battery operated radios and flashlights were present and accessible. The last fire drill was 12/19/2015. The last shelter-in-place drill was 9/16/15. Five children?s records were reviewed and in compliance. Two staff records were reviewed. Standard #: 22VAC40-185-160-A Description: Based on review of two staff records, the center did not have documentation of a tuberculosis test or screening for staff within 21 days of hire or within 12 months prior to hire. Evidence: Staff A (hired 12/15/15) did not have documentation of a tuberculosis test or screening on file. Plan of Correction: Staff A had gone to the health department who had closed their tuberculosis screenings earlier than scheduled. Staff A is scheduled to return there on January 25.
