Comments:
An unannounced monitoring inspection was conducted on January 14, 2016. Upon arrival to the facility, there were 35 children with five staff members. Additional staff were on site for administrative and support purposes. Children were observed singing, coloring, painting and playing in centers. Licensing Inspector reviewed supervision, activities, equipment, diapering procedures, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, emergency supplies and required postings. One medication was reviewed. Three children's records were reviewed. Two staff records were reviewed. The center has a total enrollment of 85 children at this time. The large wooden ship is not currently being used on the playground. Exit interview was conducted with the Program Supervisor.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, in two of three children's records reviewed, the licensee did not ensure that each child's record was documented with all required information. Evidence: During the inspection conducted on January 14, 2016, in the records for Child #1 and Child #2, there was no documentation of the name of any additional programs or schools that the child is concurrently attending and the grade or class level.

Plan of Correction: The records will be updated with that information.

Standard #: 22VAC40-185-70-A
Description: Based on record review, in two of two staff records reviewed, the licensee did not ensure that each staff person's record was documented with all required information. Evidence: During the inspection conducted on January 14, 2016, the records for Staff #1 and Staff #2 did not include documentation of job titles and written information to demonstrate that the individuals possessed the education.

Plan of Correction: I will notify staff and request documentation of education and prior training for those staff.

Standard #: 22VAC40-185-330-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that there was an adequate fall zone which encompassed a sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Evidence: During the inspection conducted on January 14, 2016 the toddler playground area, the height of the blue slide on the plastic playground equipment measured at approximately 2'6" in height. The fall zone at the end of the slide chute should be at least 6'6". There was a wooden border that measured at approximately 6' from the end of the slide chute.

Plan of Correction: The playground will be assessed to determine if we will rearrange the playground or request a variance.

Standard #: 22VAC40-185-440-J
Description: Based on observation and inspection of the facility, the staff members did not ensure that there was 30 inches between service sides of occupied cribs. Evidence: During the inspection conducted on January 14, 2016, there was an infant swing directly against an access side of an occupied crib.

Plan of Correction: The room will be rearranged today and we will remove any equipment that is currently not being used.

Standard #: 22VAC40-185-550-M
Description: Based on review, in four of four injury records reviewed, the licensee did not ensure that each record was documented with all required information. Evidence: 1. During the inspection conducted on January 14, 2016, the injury report reviewed for Child #1 was dated 11/12/15 and did not include documentation of how the parents were notified, the time the parents were notified and the staff present. 2. The injury report reviewed for Child #2 was dated 1/12/16 and did not include documentation of how the parents were notified and the staff present. 3. The injury report reviewed for Child #3 was dated 12/21/15 and did not include documentation of how the parents were notified and the staff present. 4. The injury report reviewed for Child #2 was dated 10/01/15 and did not include documentation of how the parents were notified.

Plan of Correction: I will notify all staff on the proper procedure for injury reports.
