An "other" investigation was conducted on this date due to a self-reported incident received in the Valley Licensing Office on 01/11/2016. Interviews were conducted and documentation was reviewed regarding a child being out of sight and sound supervision. There was evidence to support that a child was left out of actual sight and sound supervision.

Standard #: 22VAC40-185-340-F
Description: Based on the facility self-reporting there was a failure to ensure a child nine years old was in actual sight and sound supervision of staff. Evidence: 1. On 01/11/2016 staff #1 via e-mail notified the licensing inspector that child #1 was outside sitting on a bench not in actual sight and sound supervision of a staff member for approximately 5 - 10 minutes. 2. On 01/14/2016 the licensing inspector reviewed documentation and talked with staff #2 and staff #3 who confirmed child #1had not come inside with the rest of the children.

Plan of Correction: We implemented as of the next day writing down on paper with numbers by each name the names of the children in care. We have a symbol which indicates if a child is currently outside or inside. We also continue to count heads at least every five minutes or more frequently if there is a lot of shifting inside or outside by the children in care.
