Technical Assistance:
Kitchen safety was discussed. If the children are in the back using the bathrooms, any hazardous substances must be in locked locations. Any choking hazards, sharp utensils or other hazards must be inaccessible to the children. If the kitchen, storage area or office area are not gated, then items in these areas are considered to be accessible.

Comments:
The inspector arrived at 9:35 AM and departed at 12:00 PM. Seventeen children were in care with four staff. the program director and assistant director were also present. Morning activities, lunch service and preparation for nap time were observed. A sampling of five children's records and three staff records were reviewed. An exit meeting was conducted with the program director during which the facility's plans of correction for violations were obtained. Please contact the inspector if there are any questions.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based upon review of five children's records, the facility has not ensured that each child obtains a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: According to the record, child 4 began attending on 11/30/2015. The facility staff were unable to provide doucmentaiton of a physical examination for child 4.

Plan of Correction: Explain to parent that we need a well check up form as a physical.

Standard #: 22VAC40-185-50-A
Description: Based upon review of posted information, the facility has not ensured that staff and children's records are treated confidentially. Evidence: The Supplemental Sheet on which children and staff names are listed was posted on the parent information board at the front of the facility. The Supplemental Sheet indicates "Information found on the Supplemental Sheet is confidential and this document is not to be posted in the facility".

Plan of Correction: The sheet was taken down at the time of inspection and correct one up on board.

Standard #: 22VAC40-185-70-A
Description: Based upon review of three staff records, the facility has not ensured that each staff record contains the name, address and telephone number of a person to be notified in an emergency. Evidence: The record for staff three does not contain the address of the person to be notified in an emergency.

Plan of Correction: Make sure each staff member at the time of filling out paper work put all addresses for Contact Person. The staff member filled in the address that was missing at the time of inspection.

Standard #: 22VAC40-185-270-A
Description: Based upon a tour, the facility has not ensured that areas, inside and outside, are maintained in a safe condition. Evidence: The paint along the bottom/base of the cinder block wall of the building in the fenced playground area is peeling. A three inch strip of paint was peeled off during the inspection.

Plan of Correction: The landlord will be called to fix the peeling paint problem.

Standard #: 22VAC40-185-550-M
Description: Based upon review of injury records, the facility has not ensured that the written records of children's injuries include the type of injury. Evidence: A written injury record dated 11/09/2015 for child 6 does not include the type or description of the child's injury.

Plan of Correction: The assistant director will talk to staff members and go over the correct way to fill out the injury sheet.
