Comments:
An unannounced monitoring inspection was conducted on 1/14/16 from 2:25pm - 5:20pm. When the licensing Inspector arrived, there were 73 children ages two years old to twelve years old in care with 13 staff. A tour of the facility was conducted and children were observed doing homework, playing games, and eating snack. Records were reviewed for five children and five staff. Medication was reviewed along with the emergency preparedness plan and emergency supplies. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of five staff records, it was determined that the facility did not ensure that each staff member shall submit documentation of a negative tuberculosis screening no later than 21 days after employment, or completed within 12 months prior to employment. Evidence: 1. The record for staff #3 (date of hire 8/17/15) had documentation of a negative tuberculosis screening that was dated 9/9/13. 2. Staff #7 (Extended Care Director) confirmed the documentation of a negative tuberculosis screening was not current.

Plan of Correction: Staff #3 completed a TB screening on 1/16/16, and it will be placed in the record. All new staff will complete a TB screening within 21 days of employment.

Standard #: 22VAC40-185-70-A
Description: Based on a review of five staff records, it was determined that the facility did not ensure that a staff record with all the required information was kept for each staff person. Evidence: 1. The record for staff #2 did not contain documentation that two or more references as to character and reputation as well as competency were check before employment, and documentation of orientation training. 2. The record for staff #3 did not contain documentation that two or more references as to character and reputation as well as competency were check before employment. 3. Staff #6 (Program Director) reviewed the records for staff #2 and staff #3, and confirmed that all the required information was not available for viewing during the inspection.

Plan of Correction: The references for staff #2 and staff #3 were completed and will be placed in their record. Documentation of the orientation training for staff #2 will be placed in the record. All new staff will have all required information when employment begins.
