Technical Assistance:
Technical assistance was given regarding the clear documentation of all transactions between the licensee and the office for the Central Criminal Records Exchange of the Virginia State Police.

Comments:
An unannounced monitoring inspection was conducted on January 13, 2016. At the time of entrance there were 86 children and 16 staff members present with children. The sample size consisted of 8 staff and 5 children's records. Children were observed preparing for and eating lunch, reading books and working on activities related to learning about the five senses. Areas of non-compliance are identified on the violation notice. The inspection began at approximately 11:05 A.M. and ended at approximately 3:00 P.M.. Please complete the columns for description of action to be taken and date to be corrected for each violation cited on the violation notice, and then return a signed copy to the licensing office within 10 calendar days of receipt. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a record review and staff interviews, the licensee did not ensure that all enrolled children had placed a copy of a physical in the child's record within one month after attendance. Evidence: During the inspection on January 13, 2016, the record for child #4 (first date of attendance 9-15-2015) did not include a copy of a physical.

Plan of Correction: Information was requested from the parent the day of the visit. Effective 1/15/16 no child will be allowed to start care without all required documentation on file. DM will review quarterly for compliance.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the licensee did not ensure the diapering surface used for diapering or cleaning children, was cleaned with soap and at least room temperature water and sanitized after each use. Evidence: During the inspection conducted on January 13, 2016, a staff member in the infant 1 room was observed during the diaper changing process and the diaper changing table was not sanitized as required. The staff member sprayed the changing table with a disinfectant then immediately wiped it down. The definition of sanitized is means treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution (i.e., bleach solution or commercial chemical disinfectant) or physical agent (e.g., heat). The surface of item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution.

Plan of Correction: Infant employees will be retrained on all sanitation policies and procedures on or before 1/22/16. Center management will observe daily for compliance.

Description: Based on observation, the licensee did not ensure the tables used for eating are sanitized before and after each use. Evidence: During the inspection conducted on January 13, 2016, a staff member in the Pre-K room was observed preparing for lunch and did not sanitized the tables as required. The staff member sprayed the table with a disinfectant then immediately wiped it down. The definition of sanitized is means treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution (i.e., bleach solution or commercial chemical disinfectant) or physical agent (e.g., heat). The surface of item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution.

Plan of Correction: Pre K staff will be retrained on all sanitation policies and procedures on or before 1/22/16. Center management will observe daily for compliance.

Standard #: 22VAC40-185-560-N
Description: Based on observations, the licensee did not ensure that children were not allowed to drink or eat while walking around. Evidence: During the inspection on January 13, 2016, child #2, #3, #4, and #5 were observed walking around and eating during lunch time. The children were observed eating broccoli, barbecue chicken and bread.

Plan of Correction: Infant and toddler staff will be retrained on family style dining practices as well as safe eating policies on or before 1/22/16. Center management will observe daily for compliance.
