Comments:
An unannounced monitoring inspection was conducted on February 4, 2016 from approximately from 12:00 PM until 1:45 PM. At the time of entrance there were 16 children and 4 staff members were present with children. The sample size consisted of 3 children's records and 1 staff record. Two medications were reviewed. Children were observed during their nap time. Areas of non-compliance are identified on the violation notice. The results of the inspection were reviewed with the center designee during the exit interview on this date.

Violations:
Standard #: 22VAC40-185-510-E
Description: Based on observation and staff interviews, the licensee did not ensure all medications were labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. Evidence: During the inspection on February 4, 2016, there was medication, Ventolin for child #4 that was not labeled with the child's name, dosage amount and or times to be given.

Plan of Correction: The center designee will ensure the medication will be returned to the mother for a pharmacy label or a new medication obtained.
