Comments:
An unannounced full renewal inspection was conducted today. There were 48 children in care, with a total of 5 staff members assigned to their direct supervision. This inspection took place on a public school closure day. The children were observed eating lunch, enjoying table top games, playing in the gym, making props for their upcoming play, and rehearsing for their upcoming play production of "The Wiz." This inspection took place between approximately 12:20 pm and 1:30 pm. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: 1. Child record #3 does not include documentation to show that the parent has reviewed the contact information within the past year. 2. Child record #5 does not include a shot record.

Plan of Correction: 1. I will ask the parent to review the child's record at pick up today. 2. I will get a copy of the shot record from the school nurse.
