22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring visit and facility tour was conducted from 10:50 am-12:40 pm. At the time of inspectors' arrival there were 84 toddler-6 year old children in care with 8 staff members and 2 administrators. Children were observed participating in Montessori activities/instructional time in the classroom, transitioning between activities and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Medications were also reviewed. The director reported that there had been no injury reports since the last inspection. Areas of noncompliance are noted on the violation notice and were discussed with the Director during exit meeting at the conclusion of this inspection.
Standard #: 22VAC40-185-260-B Description: Based on record review it was determined that the center did not have the required annual approval from the health department. Evidence: The last health inspection was dated 10/17/14. The Director noted that they have an inspection scheduled for 2/3/16. Plan of Correction: The Health Department has been scheduled to perform an inspection on 2/3/16 at 10:45am.
Standard #: 22VAC40-185-270-A Description: Based on observation it was determined that the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean and safe condition. Evidence: 1. In the Children's House I classroom there was rabbit feces on the floor around the rabbit hutch adjacent to a table where children were participating in montessori lessons. 2. In the Children's House II classroom there was a roll of plastic garbage bags in the unlocked cabinet next to the sink, presenting potential suffocation hazards. 3. In the Children's House III classroom there was an apple slicer, carrot peeler and 3 loose 1" screws in an unlocked drawer next to the sink, presenting potential injury and choking hazards. All of the above noted areas were accessible to the children in care. Plan of Correction: Staff will ensure that all unsafe materials are inaccessible to children and all areas occupied by children are clean.
Standard #: 22VAC40-185-280-B Description: Based on observation it was determined that the center did not ensure that all hazardous substances are kept in a locked place using a locking method that prevents access by children. Evidence: 1. In the Children's House I classroom there were containers of disinfectant wipes in an unlocked upper level cabinet. 2. In the Children's House III classroom there were containers of disinfectant wipes, dry erase board, metal polish, fish chemicals and hand sanitizer in an unlocked upper level cabinet. 3. In the Toddler Room there was a container of disinfectant wipes and hand sanitizer in the bathroom on an upper level shelf. All of the above noted chemicals carried a "keep out of the reach of children" label. Plan of Correction: Staff will ensure that all chemicals are locked up.
Standard #: 22VAC40-185-550-E Description: Based on record review it was determined that the center did not have documentation of the required shelter-in-place drills. Evidence: There was no documentation of the two required shelter-in-place drills. The director noted that these drills were performed in April and November 2015 but they were not documented. Plan of Correction: Director will ensure that all shelter-in-place drills are documented.
