Comments:
An unannounced monitoring inspection was conducted on 2/1/16 between 3:45 and 6:00. The inspection was conducted with the director. The inspector spoke with staff and children. he following staff/child ratios were observed: One group in the front room, one staff with 14 children having a contest by balancing a book on their head. Another 30 children were in the back room with two staff. The director was also present. Later the children had dinner and watch a movie. Afternoon snack was gold fish crackers and juice. Dinner was hot dogs on buns, green beans, corn and milk. Health inspection ? 5/14/14 (see violation notice); Fire inspection ? 7/15/15; Last fire drill 1/29/16 and a shelter-in-place drill was conducted on 12/17/15 All required documents were posted including license, licensing notices, menu, and evacuation information

Standard #: 22VAC40-185-260-B
Description: The center has not had an inspection from the health inspection in over a year. The center does not cook food at this site, but does transport food from the the other site, refrigerates the food until time to serve the children either snack or dinner and then warms the food in the microwave oven. Annual health inspections are required. Evidence: When asked for the latest health inspection report, the center indicated the posted report dated 5/14/14. The director stated that she had contacted the health department several times, but that there was a new inspector who did not think that he needed to inspect this site.

Plan of Correction: Contacted health depart several times and no visit to date. I will contact again 2/2/16 by email and have documentation.
