22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 11:15am-1:30pm. Upon arrival there were 54 children in care with 11 staff present. A sample of 2 staff and 5 children's records were reviewed in addition to 4 medications. Children were observed during lunch, circle and preparing for nap. Babies were observed eating and sleeping in cribs.
Standard #: 22VAC40-185-160-C Description: Based on observation the center failed to ensure staff had a repeat tuberculosis screening at least 2 years from the date of the initial screening. Evidence: Staff #2's last tuberculosis screening was dated 11/13/12. Plan of Correction: TB Test was received by staff member on 2/5/16.
Standard #: 22VAC40-185-270-A Description: Based on observation the center failed to ensure areas and equipment of the center, inside and outside, were maintained in a safe condition. Evidence: 1. On the playground there was a blue bicycle that had missing handle bars. Plan of Correction: Bike was removed from playground on 2/5 after the visit. Staff and management will continue to monitor playground for safety issues.
Standard #: 22VAC40-185-510-A Description: Based on observation and a record review the center failed to ensure all requirements were met when medication is given. Evidence: 1. Child #4 had a prescription emergency medication that did not have written authorization to administer by the parent. 2. Child #5 had a prescription emergency medication that was in a plastic bag and did not have an original box. Plan of Correction: Prescription form was signed on 2/5/16 by the parent in the afternoon. Management will not take medication or forms until all information is completed and accurate.
