Comments:
An announced initial inspection was conducted on Friday, February 5, 2016 from 11:30am to 12:48pm. The licensee is assuming a currently licensed child day center. There were children in care during this inspection. The inspector met with the owner/director. The center consists of two large spaces. One space is for infants and toddlers and the other is for preschool and the school age before and after program. Each area is equipped with age appropriate furniture and a variety of toys and learning materials. There is also has a playground area located behind the center. The playground has playhouses and riding toys. The center is requesting a licensed capacity for 50 children ages birth through 12 years old. The program will offer meals and snacks to children in care. A weekly menu was posted No medication will be administered. The center has one van. Transportation is provided. The center has submitted all of the required application materials at this time. The center is in substantial compliance with the standards that could be determined. A six-month conditional license will be recommended. During the conditional period, the center will be subject to unannounced licensing inspections, with the first inspection taking place within the first sixty days of licensure. Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 2/5/2016, the center did not ensure that two of three records reviewed contained the required information. EVIDENCE: 1. The record of Child #1 did not contain a phone number for the place of employment listed for the father. When applicable, each child's record shall contain a work phone number and place of employment of each parent. 2. The record of Child #2 did not contain a phone number for the emergency contacts listed. Each child's record shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: Will get the missing numbers today.

Standard #: 22VAC40-185-500-B
Description: Based on observation on 2/5/2016, the center did not ensure that there was a foot operated trash can in the diapering area. EVIDENCE: The foot operated trash can in the children's bathroom with the diaper changing table was not operable. Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Plan of Correction: Will get a new foot operated trash can today.
