An unannounced monitoring inspection was conducted on 2/19/2016 from 11:30am - 11:35pm. During the inspection there were 36 children ages 11 months to five years old in care with ten staff. A tour of the facility was conducted and children were observed eating lunch, and during rest period. Records were reviewed for five children and six staff. Medication, emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s
Standard #: 22VAC40-185-440-E Description: Based on observation, it was determined that the facility did not ensure that there is at least 12 inches of space between occupied cots, beds, and rest mats. Evidence: 1. In the Preschool classroom there was a row of 5 cots where there was not at least 12 inches space between the cots in the adjacent row. 2. Staff #6 (Assistant Director) confirmed that the 5 cots did not have at least 12 inches of space between them. Plan of Correction: The classroom will be re-arranged to make adequate spacing for all of the cots.
