Technical Assistance:
1. Documentation of injuries should include the child's first and last name, no initials as a substitute.

Comments:
Conducted an unannounced renewal inspection. Observed 86 children + 10 direct-care staff. Ratios and supervision were in compliance. Children were observed washing hands, eating snack, reading, doing homework, making art projects, and playing with Lego blocks. Staff were actively engaged with the children and activities. Staff demonstrated patience when needing to repeat instructions multiple times. The cafeteria was found to be clean, safeguarded appropriately, and well supplied with toys and equipment for the children. Staff on-site had current certifications in First Aid, CPR, and MAT (medication administration). Annual fire and health inspections were current. Required documents were posted. Standard #: 22VAC40-185-510-A
Description: Based on records reviewed, it was determined that 1 of 3 medication records lacked the proper authorization. Evidence: 1. Child #2 - There was no written authorization for an Epi-Pen on-site. The written authorization for the school-year 2015-2016, signed by the parent and physician, was for an Epi-Pen Jr.

Plan of Correction: The medication authorization form and medication was returned to the parent upon their arrival and new documents were requested. All checklists will be updated and implemented to ensure all medications and corresponding authoirzations are current and valid.

Standard #: 22VAC40-185-550-M
Description: Based on records reviewed, it was determined that 1 of 4 written injury reports did not include required information. Evidence: One report did not document the date and time the parent was notified of the child's injury.

Plan of Correction: Site Management will review the accident/injury record prior to presenting it to the parent and once again before filing the report.
