An unannounced monitoring inspection was conducted today at 12:40pm. There were 41 (school-agers) directly supervised by 4 staff. The physical plant, 4 staff training records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group and individual activities. There was an abundant supply of books, toys, and materials for the children. The center was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-185-150-B Description: Based on review, the facility failed to ensure that immunization records are signed by a physician, their designee or an official of a local health department. Evidence: Reviewed 5 children's records and found that CH5's immunization record was not signed. Plan of Correction: Talked with parent about immunization will receive a signed copy.
Standard #: 22VAC40-185-60-A Description: Based on review, the facility failed to ensure that children's records were maintained with the required information. Evidence: Reviewed 5 children's records and found that CH2 did not have documentation of an address for the second emergency contact. Plan of Correction: Had parent complete emergency contact address.
Standard #: 22VAC40-185-280-B Description: Based on observation, the facility failed to ensure that hazardous substances such as cleaning materials are kept in a locked place using a safe locking method that prevents access by children. Evidence: Observed Lysol disinfectant spray and dish soap were in unlocked cabinets in Room 1. Plan of Correction: Locked cabinet and reviewed virginia state licensing procedures with all staff.
